Services at Oakleaf Personal Care Home

Our monthly fee covers not only the residents' furnished room and board, but also a broad range of other services, including:

• 24-hour professional staffing
• Assistance with activities of daily living
• Continual health monitoring and nursing care
• Housecleaning service

Below is a list of services that are available in addition to the ones listed above.

Beautician and Barber Services
Beauticians provide hair styling and cutting services for men and women at Oakleaf. Appointments may be scheduled by calling Oakleaf. Charges appear on the following month's invoice.

Billing
Payment for a resident's roo, board and care is due by the first of each month. The resident's invoice includes all ancillary charges. If payment will be delayed, please contact the administrator or accounts manager.

Clothing
We suggest that residents wear clothing that is comfortable, washable and easy to take on and off, such as sweats and jogging suits. Non-skid shoes are encouraged to help prevent falls. All clothing must be permanently marked with the resident's name. The resident's family is encouraged to help maintain the resident's closet and clothing drawers when visiting.

Linen Service, Laundry and Dry Cleaning
Oakleaf provides complete linen service. For the resident's personal clothing, family or friends make take items home for laundering, or the resident may have Oakleaf launder clothes for a monthly fee. All laundry must be marked. Clothing that requires dry cleaning must be handled by the resident's family.

Mail
Daily incoming and outgoing U.S. Postal Service is provided.

Meals
All residents receive three home-cooked, nutritious, well-balanced meals, as well as a bedtime snack, daily. We also prepare individual meals for residents who have special needs. To promote social interaction, we encourage all residents to eat in the dining room. If, for some reason, the resident is unable to come to the dining room, we provide room service. We are confident that residents and their family members will be completely satisfied with the food we serve, and we invite those who are considering Oakleaf to join us for a contemporary meal.

Meals are served at the following times:

Breakfast: 7 a.m. to 9 a.m.
Lunch: 11 a.m. to 1 p.m.
Dinner: 4 p.m. to 6 p.m.

Guest Meals
With advance notice, our kitchen can prepare meals for guests. A $3.50 charge per meal may be paid to the kitchen staff or billed on the resident's monthly invoice.

Medical Supplies and Equipment
If not otherwise provided by the resident's family, Oakleaf will provide and/or arrange for medical supplies and equipment. Items supplied by Oakleaf are on a monthly charge basis.

Medications
Oakleaf manages and administers all resident medications. All medications are stored individually in a locked, secure location. No medication, either prescription or over-the-counter, is permitted in resident rooms. Administering is by physician orders only.

Oakleaf has contracted with an area pharmacy to provide medication directly to our facility. A monthly itemized invoice of all medication will be mailed separately from the pharmacy. Residents who chose not to use this pharmacy must have all medications supplied by the family on a timely basis.

TB Testing
All residents of Oakleaf will have Mantoux Tuberculin Skin Testing annually, as suggested by the Center for Disease Control and the Allegheny County Health Department. New residents admitted to Oakleaf will be ttested. The cost of the screening is $10.00.

Money and Valuables
We encourage family members to keep all money and valuables belonging to the resident. Oakleaf is not responsible for loss of money, jewelry, clothing or other personal belongings.

Newspapers

Daily local newspaper is delivered for Oakleaf residents' reading. Individual subscriptions may be arranged by personally contacting the paper's circulation department.

Parking
Free parking is available for families and guests. Visitors are requested not to block the front entrance driveway.

Room Changes
The administrator will notify the resident's family when the resident requests or requires a room change.

Safety and Security
Beyond our focus on comfort, we maintain a commitment to safety and 24-hour security. Our building is a well-built brick and concrete structure protected by an alarm system that monitors every door. We also offer a wrist wanderguard system for those residents prone to wandering. Contributing to fire safety is a sprinkler system that runs throughout all resident rooms and common areas. All bath and resident rooms are equipped with a nurse call system.

Smoking
Smoking is permitted only in the exterior courtyard and in a designated area of the building. There are no exceptions.

Snacks
Snacks, if stored in sealed containers, are permitted in resident rooms. Snack machines are available in the dining room. Because many residents have dietary restrictions, we ask that family and friends consult our director of wellness when there are concerns.

Telephones
Private phone line installation can be arranged by calling Curry Communications at 412-702-1717. Telephones must be provided by the resident's family. Residents may also use the public phone conveniently located in the hallway.

Televisions and Radios
Three cable television lounges and one radio lounge are located throughout the facility. Private televisions and radios are permitted in resident rooms. Resident's family can arrange for private cable service by calling AT&T Cable Services at 1-800-532-3779.

Toiletries
The resident, or the resident's family, is responsible for purchasing toiletries. Toiletries may be supplied by Oakleaf on a charge basis.

Transportation

Oakleaf will arrange for emergency ambulance services. The ambulance company will bill the resident's responsible party.

Transportation for hospital visits and routine physician appointments should be made by the family via Access, OPT or ambulance transport services. Oakleaf is serviced by Baldwin Emergency Medical Service and residents may subscribe to individual memberships by calling BEMS at 412-884-0666. Charges not covered by insurance are the responsibility of the resident. Oakleaf can provide escort service for other medical or professional appointments on a charge basis.

Vacation/Change of Address for Responsible Party
Individuals and families responsible for residents are asked to notify the Administrator or the Director of Wellness of an anticipated vacation, or change of address or telephone number. These changes will be placed on file in case of emergency.

Visits and Departures
We encourage family members and friends to regularly visit Oakleaf and continue to include them in outings and get-togethers. There are no age restrictions for visitors. Small pets on leashes are also welcome. If possible, we ask that visitors refrain from visiting during meal times. Residents who wish to leave for a short time may be signed out by an approved adult by advising the shift supervisor.

Note: Some items or services may be covered by Medicare, Blue Cross or other third party payers. All insurance arrangements are made by the resident's family. Those not covered by insurance are the financial responsibility of the resident or resident's family.
