Admission Policy

Each prospective resident must have completed the following at the time of admission:

1. Medical Evaluation

A medical evaluation completed by his her primary care physician on a resident medical evaluation form provided by Oakleaf.

2. Pre-Admission Screening

A pre-admission screening by the Director or Assistant Director of Resident Care to evaluate appropriateness for personal care.


3. Needs Evaluation
Oakleaf's Administrator or Director of Resident Care will evaluate a prospective resident and admit only individuals whose care and physical needs can be provided by the staff and home. Prior to occupancy, pertinent information regarding emergency and ancillary services will be required.

This includes:


• An attending physician will be confirmed--either the house physician or individual primary care physician.
Proof of age.
• Copies of residents' insurance cards and other identification cards, including Social Security, Medicare, Medicaid, PACE, and Access.
• Copies of Living Will, Power of Attorney, advanced directives, and/or guardianship.
• Payment for the first month, plus the security deposit at the time of admission.

Oakleaf's criteria for admission or re-admission from a hospital include the following:

• Resident Medical Evaluation Form
• Discharge Summary
• History and Physical
• Therapy Notes
• Pharmacy Prescriptions

Admission is preferred before 11 a.m., Monday through Friday, unless special arrangements have been made with the Administrator or Director of Resident Care.

No admission will be accepted without:
• Completed admission forms
• Appropriate payment
• Designated primary care physician


Discharge Policy

Unless prior arrangements have been made with administration, residents must provide Oakleaf 30 days written notification to terminate a residency. The exceptions are for hospitalization that requires a change in the level of care or death. In such situations, a refund will be processed.


Non-Discrimination in Services Policy Statement


Admissions, the provision of services, and referrals of residents shall be made without regard to race, color, religious creed, disability, ancestry, national origin (including limited English proficiency), age, or sex.

Program services shall be made accessible to eligible persons with disabilities through the most practical and economically feasible methods available. These methods include, but are not limited to, equipment redesign, the provision of aids, and the use of alternative service delivery locations.

Structural modifications shall be considered only as a last resort among available methods.

Any resident (and/or their guardian) who believes they have been discriminated against, may file a complaint of discrimination with:
